• I am over the age of 18, contractually capable of completing this application and agreeing to the indemnity outlined in the attachment
• The contents of this application are true and correct
• I have read and understood that contents of the indemnity and confirm in favour of the Club that I accept the terms thereof
• I agree to abide by the Constitution and Rules of the Club as they are adopted and amended from time to time
• If applying for membership as a tandem passenger, I have read and understood the contents of the Experimental Test Parachute Jumper Assumption of Risk Agreement, and confirm in favour of the Club that I accept the terms thereof
